Hello.
I've been seeing this panic since the beginning of this month; it's not
the panic on vkernel itself, but the DragonFly kernel running the vkernel
executable. Basically what I have to do to reproduce the panic is to run
buildworld on vkernel (sometimes buildkernel is enough, sometimes not).
I used -n16 on vkernel's command line argument, but probably I could trigger
the panic with a smaller number.
The first version which triggered the panic was 376534279, one version
earlier than the pmap_map() syntaxis change. The backtrace below was
from the kernel built from the source tree as of a307f7025.

I see the syntaxis change but I don't see any version 376534279.
Are you saying the problem occurred before the syntax change was
committed? Or after?

Are you saying the problem occurred before the syntax change was
committed? Or after?

Before the syntaxis change was committed, that is, the panic occurs
on the real kernels built from both versions below:

Ah, ok.  I was only git log'ing /usr/src/sys/, 3765 was a commit
in the main source tree :-)
The panic is due to pmap->pm_stats.resident_count being off by 1.
It should have been left with a count of 1 with only the page
directory page left to purge but the count was 0.
I'm thinking possibly something in the pmap unwiring code, possibly
in _pmap_unwire_pte_hold(), could be racing.
Here is a patch to try.  It adds a bunch of assertions in an attempt
to catch the potential race.

That's a better panic, for sure.  Here is another patch to try:

  • I fixed one place where pmap->pm_generation was not being bumped.
  • I fixed another place where it was looping on the wrong npv
    under certain circumstances (but it also kprintf()'s something
    in that case and your crash dump didn't have the kprintf).
  • I added a few more KKASSERTs closer to the pmap lookup.
  • I believe I found a race in pmap_enter()'s call to pmap_remove_pte().
    The temporary ptbase can get wiped after the call so we have to reload
    the pte pointer.
I'm crossing my fingers.  If this fixes the problem I will make the
same changes to the x86_64 and vkernel's pmap code.

The crash dumps are definitely providing more information now.
It sees a pte entry for which there is no corresponding pv.  The
page is mapped to multiple pmaps (but not the particular one that
we paniced on), its a vkernel virtual process stack page, so it
was fork()ed a few times.... hence why more cpus can reproduce
the race.
We caught the problem earlier this time.  If it had been allowed
to go through it would have cleared the pte entry and then later
on paniced when it found an zero pte entry. I think there are
two possibilities.
  • The first is that the pmap system is allocating a page marked
    PG_ZERO which isn't actually an empty page, then encountered
    the junk pte later on.
  • The second is that there is another race where the temporary
    page table mapping gets blown up and causes a pte to be entered
    into the wrong pmap, then later we encounter the bad pte and
    can't find its corresponding pv.
Here's a new patch to try.  I added a bunch more assertions to
try to catch it and I also check to make sure PG_ZERO'd pages
are zero (which is expensive but...).

It's a bit different.  In this dump I found that for the entry
that asserted the pv was missing from the vm_page_t's pv list
but was present in the pmap's list for that va.
Since the pv is added and removed from both lists in tandem
that shouldn't be possible.
So... more debug code in this next patch.  Now I verify that
the pte being added or removed matches the vm_page_t being
associated with the pv entry. Here is the patch:

or bad timing if you've accelerated your cpus. Try shifting
the memory around and detuning the cpu frequency or FSB frequency
if the BIOS has the ability to do so. I only give it a 15% chance

Never mind on the bad memory possibility... I just reproduced it on
my test box using a vkernel -n 8 and a buildworld -j 8.

Although the CPU is not over-clocked, I have three memory modules of
512Mbytes with different speeds(DDR266, DDR333, DDR400) installed
on this system. I believed this should be no problem; the BIOS
screen indicated that all modules are treated as DDR266. But I'll try
with the DDR400 one and see if that helps (I'm not sure 512M is enough
for HAMMER and vkernel, though).

I think I found it. It's a bug in zalloc, in vm/vm_zone.c.

Remove all the other patches and just throw in that one.  The pmap
code uses zbootinit() and zinitna(). zbootinit() takes a fixed
item array and uses the structure size as-is. zinitna() rounds-up
the structure size to a 32 byte boundary. The pv_entry structure
is 28 bytes.
That blows up the calculation when the zone is expanded and creates
an overlap, thus corrupting one of the pv_entry's.
I found it when I added another field to pv_entry and suddenly the
panic stopped occuring on my test box.
There are still a few bugs in pmap.c which I will commit from the
other patches. As soon as I verify that the bug is in zalloc
I will clean up the pmap.c patch and commit it along with the zalloc
fix.
OMG this one was hard to find.  Of course, I haven't verified that

The same panic isn't triggered anymore. I can't tell exactly which commit
fixed it, but I guess the pmap-related commits starting from 5926987a
at the end of year 2009 were the ones.
